It was established through a partnership involving the National Institute on Alcohol Abuse and Alcoholism (NIAAA), the Substance Abuse and Mental Health Services Administration (SAMHSA), and the nonprofit Screening for Mental Health, Inc. A 2001 NIAAA announcement describes the program as an opportunity to explore whether alcohol may be causing problems and connect with helpful resources.

National Alcohol Screening Day programs have included free, anonymous screening questionnaires, educational information, conversations with health professionals, and referrals for further evaluation or treatment. Screening sites have operated in settings such as colleges, hospitals, clinics, and community centers. The Alcohol Use Disorders Identification Test (AUDIT), a 10-question screening scale, has been used to identify people who may be drinking at hazardous or harmful levels. The day also aims to make it easier to discuss alcohol-related concerns without stigma.
